New JAMA Review Highlights Active, Conservative Care for Low Back Pain

NEW JAMA REVIEW HIGHLIGHTS ACTIVE, CONSERVATIVE CARE FOR LOW BACK PAIN

STUART, FL — A new clinical review published in JAMA provides updated guidance on the evaluation and treatment of nonspecific low-back pain, one of the leading causes of disability worldwide.

The review reports that approximately one in four U.S. workers experiences low-back pain. Although back pain can significantly interfere with work, sleep, exercise and daily activities, most patients seeking care do not have a fracture, infection, cancer or another serious spinal condition.

One of the review’s most important messages is that patients should generally remain active rather than relying on prolonged bed rest. Education, appropriate movement and a gradual return to normal activities can play an important role in recovery.

For acute nonspecific low-back pain, the review identifies conservative options such as heat, spinal manipulation, massage and acupuncture. For chronic low-back pain, the recommendations emphasize exercise, self-management, psychologically informed care and coordinated multidisciplinary treatment. Spinal manipulation and massage may also provide modest short-term benefits for appropriately selected patients.

The review also advises against routine X-rays, CT scans or MRI examinations for uncomplicated nonspecific low-back pain. Imaging may be appropriate when a patient has severe or progressive weakness, saddle numbness, urinary retention, significant trauma or symptoms suggesting fracture, infection, cancer or another serious condition.
